Privacy Policy
This page explains what personal and health information this service collects, why, how long it's kept, and your rights over it. It applies to mdconsult.in and the online consultation service run by Dr. Ashwin Rajenesh. It sits alongside, and doesn't replace, the privacy terms in the consent document you sign before your first consultation — that consent document is what governs use of your information for your clinical care specifically.
What we collect
- Identity and contact details: full name, date of birth, sex, nationality, address, phone number, email address.
- For a patient under 18: a parent/legal guardian's name, relationship, and contact details.
- Health information: your medical history, current symptoms and medications, allergies, physical examination findings (including photos/videos where a self-submitted exam applies), investigation and lab reports, working diagnosis, treatment plan, and prescriptions.
- Records of your interactions with this service: appointments, consent records, symptom-tracker entries, and correspondence.
Why we collect it
Solely to provide your medical care: to register you, take your history, carry out and document your consultation and examination, review your reports, arrive at a diagnosis and treatment plan, manage your appointments and follow-up, and maintain the medical record we're required to keep.
Who can see it
Your record is accessible only to Dr. Ashwin Rajenesh. Photos/videos submitted for a self-examination are held in a separate, access-controlled digital repository outside this application, for the same purpose. We don't sell or share your information with third parties for marketing, and don't disclose it beyond your care except where required by law.
How long we keep it
Your medical record is retained for as long as applicable medical record-keeping requirements call for, and for as long as you remain a patient of this practice. If you'd like your record deleted outside of that requirement, contact us and we'll explain what can and can't be removed.
Your rights
- Request a copy of your medical record and the personal information held about you.
- Ask us to correct information that's inaccurate or out of date.
- Withdraw consent to online/tele-consultation at any time, as set out in your consent document.
- Raise a question or complaint about how your information is handled.
